Let’s Talk About Sex and Matters of the Heart

No, not that kind of sex (though we do love to talk about sexual health here at Northwest Wellness Center). Today, we’re talking about the other kind — biological sex — and how it affects your heart.

Get ready, because what you’re about to learn might make your blood pressure rise (and not just from perimenopause!).

The (Gender) Gap No One’s Talking About

Here’s a shocking fact: one in three women will die of heart disease.

Yet, most cardiovascular research has been done on… men. Specifically, white men. For decades, the “average male heart” was considered the template for all of us, leaving women’s unique physiology out of the picture. Women were not even required to be enrolled in scientific studies until the 1990s!

That means that women’s symptoms — like anxiety, jaw pain, shortness of breath, or even indigestion — often get brushed off or misdiagnosed. Meanwhile, a man with textbook chest pain gets whisked straight to the cath lab. Sound familiar?

Hot Flashes, Cold Facts

Here’s one study that caught my attention: women who experienced multiple hot flashes per week had greater carotid artery narrowing (CIMT) than women who had just one per week. Translation? Hot flashes aren’t just uncomfortable — they may be telling us something about what’s happening inside the arteries and an early predictor of heart disease to come.

As arteries stiffen and narrow, the risk for heart disease and stroke climbs. And perimenopause and menopause — with the loss of estrogen — accelerate that process. Women who had normal blood pressure, cholesterol and blood sugar levels pre-menopause suddenly find themselves with rising blood pressure, cholesterol and insulin resistance in perimenopause and extending into menopause, even without changing her diet or exercise routine. Estrogen influences every cell and organ system in the body.

Estrogen isn’t just a reproductive hormone; it’s also a vascular superhero. It keeps arteries flexible, supports nitric oxide production, and protects the inner lining of blood vessels (the endothelium). When estrogen drops, arteries lose their elasticity — blood pressure goes up, and inflammation sneaks in and plaque begins to form – years before the silent killer of heart disease rears its ugly head.

It’s Not Just About Cholesterol

High cholesterol gets a lot of blame, but it’s only part of the story.
  To build plaque inside the arteries, you need three culprits working together:

  1. Endothelial damage (from inflammation, high blood pressure, or elevated blood sugar)
  2. Inflammation (that chronic, smoldering kind)
  3. Elevated cholesterol (particularly the small, sticky LDL particles)

Most conventional doctors will check a basic lipid panel — LDL, HDL, triglycerides, total cholesterol — and call it a day. But that doesn’t tell the full story.

At Northwest Wellness Center, we dig deeper.
  We look at your genetics, family history, lifestyle, and in-depth labs and imaging such as a comprehensive cholesterol analysis which may influence:

  • LDL particle size and count – small, dense LDLs are more atherogenic (plaque-forming) than the large ones which tend to bounce off the arterial wall.

  • ApoB – a marker of how many atherogenic particles are floating around looking for trouble.

  • Lp(a) – a genetically driven risk factor that can triple heart risk, and most people have never heard of it. It should be checked at least once in a lifetime but cardiologist and women’s health advocate in the menosphere Dr. Jane Morgan advocates for checking Lp(a) at least three times in a woman’s reproductive cycle – pre, peri, and after menopause and we don’t actually know (as in the studies have not been done) to know if it changes as women’s hormones change.

  • ApoE genotype – your genetic blueprint for how your body handles fats, cholesterol, and inflammation.

  • Cardiac inflammatory markers – such as hs-CRP (high-sensitivity C-reactive protein), Lp-PLA2 (lipoprotein-associated phospholipase A2), and myeloperoxidase (MPO). These tell us if your arteries are inflamed, unstable, or prone to rupture — often before symptoms ever appear.

  • Omega-3 to Omega-6 ratio – a measure of your body’s internal “inflammation balance.” A healthy ratio (closer to 1:1 or 1:2) helps protect the arteries, while too many omega-6s (common in the Standard American Diet) promote inflammation and clotting.

  • Homocysteine – an amino acid linked to endothelial damage and oxidative stress. Elevated levels increase your risk of heart attack, stroke, and even dementia — and can often be improved through B-vitamin support and methylation optimization.

  • CIMT (Carotid Intima-Media Thickness) – a noninvasive ultrasound that measures the thickness of your carotid arteries. It’s like a window into your vascular age — showing early signs of arterial plaque years before a heart attack or stroke occurs.

  • Coronary Artery Calcium (CAC) Score) – a specialized CT scan that detects calcium buildup in the heart’s arteries. Think of it as a “plaque scorecard.” The higher your calcium score, the greater your risk — but the beauty is, we can track improvement with prevention and lifestyle intervention.

And because your brain’s blood vessels are the same as your heart’s, these same risk factors can influence dementia — which is now being called Type 3 Diabetes.
